Job Description:

IHS is expanding its supportive Clinical Services teams in the community. We need mission driven team players with Case Management, Housing Navigation and/or Outreach experience to provide services to new clients. Share in IHS's legacy of service to at-risk and homeless individuals!Responsibilities:

  • Create and offer tailored solutions for those in crisis, and nurture homeless people toward greater self-direction and responsibility.
  • Provide supportive counseling, case management, outreach (when appropriate), and provision of safe, decent and affordable housing opportunities.
  • Provide skills teaching, family support, service coordination/advocacy, and linkage to community-based services.
  • Provide crisis support; work collaboratively with the treatment team to implement enrollment to HMIS/VI-SPDAT, assessment at admission/at discharge when appropriate.
  • Document to contract and IHS standards of documentation and release of information.Requirements:
    • Working knowledge of the case management process, assessment, planning and coordination of services.
    • Knowledge of community resources, levels of care, as well as the standards for care within the community resources.
    • The ability to assess physical and psychological factors impacting the case and how to coordinate service delivery.
    • Possess personal qualities of integrity, patience and commitment to mission.
    • Flexible and able to multitask; can work within an ambiguous, fast-moving environment.Required Education and Experience:
      • Minimum HS Diploma or GED equivalent.
      • At least one year of direct case management or supportive services providing linkage to community resources including housing.
      • One year of experience working with homeless or other disenfranchised individuals with mental health and/or substance abuse issues.
      • Valid Drivers License with clean traffic abstract.Strongly Preferred Education and Experience:
        • Associates or Bachelor's Degree in Human Services, Social Work, Psychology or related field with 12 semester credit hours of sociology, psychology, human services etc.
        • Two years of direct case management or supportive services.
        • Two years of experience working with homeless or other disenfranchised individuals with mental health and/or substance abuse issues.
        • Valid Drivers License with a clean traffic abstract.
