As navigators, the Navian Ohana provides clarity and guidance through times of uncertainty. As guardians, they provide healing and compassion.Primary Responsibilities:Patient Care

  • Implements plan of care developed by Facilities/Case Manager
  • Conducts individualized focused patient assessments; collecting data based on the patient's immediate condition or needs within timeframe specified by Hospice Hawaii's policies, procedures or protocols
  • Performs appropriate treatments as ordered by physician in an accurate and timely manner
  • Manages crisis situations in a professional manner
  • Performs therapeutic nursing interventions as established by individualized plan of care to the patient and/or family
  • Provides individualized patient/family education customized to the patient/family
  • Documents patient assessment findings including physical/psychosocial responses to nursing intervention and progress towards pain/symptom management
  • Delegates and supervises care responsibilities within the individual's scope of practice and/or other regulatory parameters
    • Maintains confidentiality in matter related to patient, family and client facility staff
    • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient/family diversity, preserving their autonomy, dignity and rights
    • Reports patient condition to appropriate personnel during each shift - i.e. changes in patient condition to case manager, medical director, patient's physician
    • Works in conjunction with the Social Worker and Case Manager of the facility to implement a specific plan of care for each patient
    • Care provided reflects compliance with Federal and State regulations, along with Hospice Hawai'i, Inc. policies and proceduresKnowledge and Skills Requirements
      • Demonstrates knowledge, attitudes, behaviors and skills that are consistent with the scope of practice for hospice and palliative LPNs
      • Ability to establish and maintain effective working relationships with staff, patient/family members, the public, and representatives of other agencies and organizations
      • Ability to communicate effectively and professionally
      • Ability to handle crisis situations in a professional manner
      • Strong customer service skills and helpful attitude
      • Ability to identify problems/issues/concerns and actively participate in resolution of problems
      • Organized, able to manage multiple tasks at a time, and prioritize tasks
      • Ability to maintain records and prepare related reports
      • Familiar with concepts and practices used in a hospice setting
      • Maintains physical, social, emotional, and spiritual care of oneselfExperience and Educational Requirements:
        • Licensed Practical/Vocational Nurse with current license in good standing to practice as a licensed practical nurse in Hawaii.
        • At least two years experience as a staff nurse in an acute hospital. Hospice and/or home health nursing experience preferred.
        • CPR BLS CertificationEmployment Requirements:All offers of employment at Navian Hawaii are contingent on a candidate's successful completion of certain requirements which include:
          • A legally compliant background check
          • Drug Screen & Physical Exam
          • Navian Hawaii has also adopted a policy requiring all associates to be fully vaccinated against COVID-19, subject to limited exemptions based on medical conditions or sincerely held religious beliefs.
          • TB ClearanceHours:
            • 12-hour shifts (6:30 am through 7:00 pm or 6:30 pm through 7:00 am). Position requires weekends and holidays availability.Job Type: Full-time
