Job Description:
Scope of Service Acute Hemodialysis operates a six bed inpatient
unit, as well as having the capability of providing treatments at
the bedside. Hours of operation are Monday through Saturday 6:30 am
to 11:30 pm. Emergency treatment is available 24 hours/day, using
on-call RNs. The average daily treatment schedule is 45. -Acute
Hemodialysis provides care for all adult patients over 18 years.
Our patient population includes: --- - Inpatients who develop acute
renal failure; --- - Patients admitted in acute renal failure; ---
- Chronic hemodialysis patients admitted for treatment of other
medical/surgical diagnosis; --- - Patients admitted with acute
overdose. -Patient care includes: --- - All medical/surgical;
including ventilator-dependent patients, stable telemetry, and
lower level of care (SNF/ICF) patients receive their treatments in
the hemodialysis unit. RN staff/patient ratio 1:2. Exceptions:
Respiratory Isolation (TB). --- - Continuous EKG monitoring/pulse
oximetry are available in the hemodialysis unit. --- - All ICU and
unstable telemetry patients receive their treatments at the bedside
by an RN (1:1). The general ICU care is provided by the ICU RN
assigned to the patient in consultation with the HD RN for HD
treatment. --- - All critical care patients are assessed per
critical care routine and the needs of the patient. --- - The ANA
Standards of Clinical Nursing Practice and American Nephrology
Nurses Association Clinical Nursing Practice Standards are
followed. Responsibilities I. JOB
SUMMARY/RESPONSIBILITIESResponsible for coordinating clinical care
of patient. - Integrates all health care providers to ensure high
quality patient/family outcomes, and establishes a consistent and
accountable relationship with patient/family. Key responsibilities
include, but are not limited to, initiation and development of the
plan of care, patient/family education, utilization management and
discharge planning.II. TYPICAL PHYSICAL DEMANDS -Essential
functions- Continuous: lifting up to 1/2 lb., seeing, hearing,
talking, grasping, fingering. - Frequent to continuous: standing,
walking, stooping, forward reaching, handling. - Frequent: pushing
and pulling up to 30 psi, lifting up to 10 lbs., sitting, bending.
Occasional to frequent: squatting, twisting. - Occasional: pushing
and pulling up to 70 psi; - lifting: - horizontal lift/carry up to
75 lbs. with assistance, floor to waist up to 35 lbs., waist to
shoulder up to 25 lbs., overhead level up to 15 lbs.; - carrying up
to 35 lbs, running, climbing, side bending, - kneeling, crouching,
overhead reaching. Operates various machines/tools/work aids.III. -
- - - TYPICAL WORKING CONDITIONSInside work. - Exposure to body
fluids, blood products, and communicable diseases. - Work
environment may be highly stressful, fast paced and hectic. -
Assignment to inpatient adult psych areas and Family Treatment
Center includes occasionally accompanying patients outdoors. -
Management of confused/disoriented and combative patients may
involve tasks which may compromise physical safety.IV. MINIMUM
QUALIFICATIONSA. Education/Certification and Licensure:1.Licensure:
- Registered Nurse, State of Hawaii2.Diploma or Associate Degree in
Nursing; Baccalaureate Degree in Nursing preferred.3.Current
BLS/HCP card in all areas; ACLS, NALS and/or PALS in designated
areas.4.Ability to read, write, speak and understand English
effectively. Additional Requirements Additional Requirements for
Experienced RN: A. - -EDUCATION/CERTIFICATION AND LICENSURE: --- -
Current ACLS certification. - B. - -EXPERIENCE: --- - 12 months
Telemetry and/or medical/surgical RN experience in the last 5
years. --- - 12 months ICU experience preferred. --- - 12 months of
acute Hemodialysis RN preferred. --- - Baxter/Phoenix HD machine
