Under the general direction of the Director of Quality Management, or designee, performs case management through direct referrals and reviews of medical records. Determines the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services. Performs treatment plan assessment, discharge needs interviews, and planning/coordination for high risk cases. Facilitates and participates in organizational quality improvement activities. Minimum Qualifications: Must meet education requirements for Hawaii state licensing/current Hawaii RN license. Three years RN Case Management in hospital setting.
