Job Description:
Job Description Job Description Step into a pivotal role as a
Registered Nurse, Case Management in Honolulu, Hawaii, where your
clinical expertise and compassionate advocacy combine to create
seamless, patient-centered care journeys. In this role, you will
assess complex clinical and psychosocial needs, coordinate across
multidisciplinary teams, and lead discharge planning to ensure safe
transitions from hospital to home or alternative care settings.
You’ll partner with physicians, social workers, therapists, and
community resources to optimize outcomes, reduce unnecessary
utilization, and empower patients and families to navigate care
with confidence. This is a chance to leverage your specialization
in case management to make a tangible difference in people’s lives
while living in one of the most culturally rich and scenically
stunning destinations in the United States. Hawaii’s beauty is an
ever-present backdrop to your work, offering a unique balance of
island serenity and professional purpose, where each day can begin
with a sunrise over the Pacific and end with opportunities to
explore volcanic landscapes, rainforests, and pristine beaches. And
Separately, you’ll find an abundance of natural and cultural
experiences across the state, from the world-famous welcome of
Waikiki to quieter coastal towns, ensuring your professional
journey is as rewarding outside the hospital walls as it is inside
them. Location Benefits: Honolulu blends a vibrant urban landscape
with easy access to nature, creating an exceptional environment for
healthcare professionals who value both professional growth and
quality of life. You’ll enjoy a stable, opportunity-rich setting
with access to advanced care networks, academic affiliations, and
resources that support comprehensive case management practice. The
opportunity to work in Hawaii offers a unique blend of population
health needs, diverse communities, and evidence-based care
approaches in a tropical climate that invites outdoor wellness and
resilience. For those drawn to broader horizons, the role also
presents the option to engage in similar assignments across various
locations throughout the U.S. This flexibility can broaden your
professional perspectives, expand your network, and expose you to
different care delivery models, payer landscapes, and patient
populations—all while maintaining the security of a structured
contract with guaranteed hours and consistent compensation. Role
Specifics and Benefits: In this multi-week assignment beginning
03/02/2026, you’ll be anchored by a guaranteed 40.0 hours per week,
with a competitive weekly pay range of $2,267 to $2,375, reflecting
the value you bring as a skilled RN in case management. Your core
responsibilities will include comprehensive patient assessments to
identify clinical needs and social determinants of health;
coordinating with inpatient teams, discharge planners, primary care
providers, and community resources to create efficient, safe
transition plans; conducting utilization review to ensure
appropriate length of stay and resource use; developing
individualized care plans that align with payer requirements,
evidence-based guidelines, and patient goals; facilitating
communication across departments to prevent fragmentation of care;
educating patients and families about chronic disease management,
medication reconciliation, follow-up appointments, and red flags
that require escalation; and meticulously documenting assessments,
plans, and outcomes in electronic health records to support quality
improvement and data-driven decision-making. You’ll have
opportunities for professional growth within Case Management, with
pathways to advance to senior case manager, care coordination
program leadership, or nurse navigator roles, all supported by a
framework of continuing education, certifications (such as CCM or
RN-BC), and mentorship. The package emphasizes not only
competitiveness but also meaningful support. You’ll receive housing
assistance to help offset temporary lodging as you transition to
Hawaii, and the potential for contract extensions based on
performance, program needs, and your evolving scope of practice. A
completion or performance bonus may be part of the offering,
recognizing your contributions to patient outcomes and care
efficiency. The role also comes with robust support, including 24/7
access to company resources and a dedicated travel support team
that assists with onboarding, credentialing, schedule changes, and
travel logistics. This comprehensive framework ensures you’re never
navigating complex cases or travel demands alone, allowing you to
focus on delivering exceptional patient-centered care while
adapting to diverse care settings and payer requirements. Company
Values: Our organization is committed to empowering our nursing
staff through clear career development paths, ongoing education,
mentorship, and a culture of collaboration. We believe that
supportive leadership, access to resources, and a respectful work
environment drive professional growth, enhance patient outcomes,
and contribute to a fulfilling, long-term career. You’ll join a
team that recognizes your expertise, values your input, and
coordinates with you to build resilient care plans and improved
transitions—an environment where your voice matters and your
professional progression is supported. Call to Action: If you’re
