Job Description Job Description About the Role: The Family Nurse
Practitioner (FNP) plays a critical role in delivering
comprehensive healthcare services to patients across the lifespan,
from infants to older adults. This position focuses on providing
primary care, managing acute and chronic illnesses, and promoting
health education to improve patient outcomes. The FNP collaborates
closely with physicians, specialists, and other healthcare
professionals to develop and implement individualized care plans. A
key outcome of this role is to ensure patients receive timely,
evidence-based medical interventions while fostering a supportive
and empathetic environment. Ultimately, the FNP contributes to
enhancing community health by addressing diverse patient needs in
various clinical settings. Minimum Qualifications: Current and
valid Family Nurse Practitioner (FNP) certification and licensure
in the state of Hawaii. Unencumbered DEA license. Registered Nurse
(RN) license in the state of practice. Master’s or Doctoral degree
in Nursing from an accredited program. Experience delivery of
primary care role with 3 years of experience. Basic Life Support
(BLS). Preferred Qualifications: Experience in primary care with
adult population. Familiarity with Suboxone for opioid use
disorders. Previous work experience in urgent care, intensive care
unit or outpatient clinical settings. Familiarity with electronic
health record (EHR) systems and telehealth platforms.
Responsibilities: Conduct thorough patient assessments including
history taking, physical examinations, and diagnostic testing.
Develop and implement treatment plans for acute and chronic
conditions, including medication management and follow-up care.
Provide patient education on disease prevention, health
maintenance, and management of chronic illnesses. Administer and
manage primary care protocols as appropriate within scope of
practice. Respond to urgent care needs and coordinate referrals to
specialists or higher levels of care when necessary. Maintain
accurate and detailed patient records in compliance with healthcare
regulations and standards. Collaborate with interdisciplinary teams
to optimize patient care and participate in quality improvement
initiatives. Skills: The required skills such as patient education
and medical/surgical nursing are essential for assessing patient
needs and delivering effective treatment plans daily. Proficiency
in primary care allows the FNP to support patients with complex
conditions, ensuring comfort and adherence to treatment protocols.
Critical care and urgent care skills enable the practitioner to
respond swiftly to emergent health issues, stabilizing patients and
coordinating further care. Collaboration and communication skills
are vital for working within multidisciplinary teams and educating
patients and families about health management. Additionally,
familiarity with electronic health records and clinical
documentation supports efficient workflow and compliance with
healthcare standards.
