The Villas in Honolulu, Hawaii is looking for a Social Services Associate to help us with sourcing, locating, and recruiting talented, best qualified, and culture-add candidates who embrace Ohana's mission and values.

Shift:

Duties include but are not limited:

  • In conjunction with the Social Services Manager, identifies and assesses upon admission the guest's psychosocial, emotional, environmental, spiritual, and support needs in order to assist in developing a plan of care designed to meet that guests/guestsindividual needs. Involves the guest and/or family in the plan of care.
  • Assists with counseling and crisisintervention to facilitate effective coping/problem solving strategies in relation to illness/disability, death, and/or emotional and relationship problems.
  • Assists with management of the PASRR process and tracking level of care changes and related paperwork.
  • Makes daily rounds to all units and reviews communication logs, social service logs and loss logs.
  • Completes portion of admission agreement with new guests and their families.
  • Completes theprocess for Social Services toinclude:initial assessments, state/federal timed assessments,andcare plans.
  • Documents services and guest care activities according to established policy and procedures to assure timely and accurate completion of records.
  • Involves guest and family in the discharge planning process.
  • Coordinatesand attends meetings, along with taking minutes.
  • Receives, investigates grievances, coordinates and attends family and guest conferences for grievances as necessary.
  • Maintains guest needs requests and shops for all guests requiring items.
  • Identifies guests who need to Obtain POA, Surrogates or Guardianship and assists in coordinating process to ensure all guests have a representative on admission or within 1 month of admission.
  • Assists with development, revision of current forms and department policies and procedures in accordance with facility procedures.

Requirements:

  • CPR and First Aide Certifications required.
  • Current Hawaii drivers licensed required.
  • Bachelor's degree in Social Work preferred.
  • Membership in National Association of Social Workers and Academy of Certified Social Work preferred.
  • Licensure/ Hawaii State Social Work license or eligible preferred.
  • Minimum of 1yearofexperience providing social services or in long term care preferred.
  • Knowledge of medical terminology preferred.
  • First Aid and CPR preferred. (can be done in-house).
  • Must have current medical/health screening including 2 step TB clearance (can be done in-house).
