Program Location: Sultan Early Intervention, Honolulu, HI Program Hours: Monday through Friday, 8:00a - 5:00p, occasional weekends may be required based on clients service plan or as requested Program Schedule: Flexible start time to be discussed and determined with Program Manager Report To: Program Manager Status: Full-Time, Regular, 20 Hours/Wk; Work schedule within business hours of Monday - Friday, 8:00am to 5:00pm with flexibility EASTERSEALS HAWAII 'S MISSION STATEMENT Easterseals Hawaii (ESH) provides exceptional, individualized, family-centered services to empower people with disabilities or special needs to achieve their goals and live independent fulfilling lives. JOB SUMMARY As a member of a multidisciplinary team, provide immediate interim and on-going culturally competent, family-centered care coordination services for infants and toddlers with special needs and their families. This position is under the direct supervision of the Early Intervention Program Manager. ESSENTIAL JOB FUNCTIONS Daily Responsibilities: * Adheres to all Department of Health - Early Intervention Section (DOH- EIS) and ESH guidelines. * Coordinates referrals and completes intake with families. * Participates as an evaluator and member of a multidisciplinary team that determines eligibility for Early Intervention (EI) services. Supports the child and family through the evaluation process. * Conducts, interprets, and shares information from Social Work or bio-psycho-social assessments. * Provides Social Work services, as needed. * Facilitates the development of the Individualized Family Support Plan (IFSP) with the family and other team members. * Assists the team and family identify, develop, and implement measurable, developmentally appropriate outcomes and objectives that meet the needs of the child and family. * Supports the family in accessing and utilizing services identified in the IFSP and links the child and family to appropriate resources and services. * Supports services that are conducted in a familys natural environment and that support a familys daily routines; educates families on the importance of the service delivery model; supports other members of the team in responding to urgent family matters as well as family and community dynamics that impact services and their childs development. * Establishes and maintains cooperative and collaborative relationships with families and community agencies, public and private. * Acts as the primary liaison for the program and family. Provide guidance to families to empower them to advocate for their child. * Schedules, coordinates, and facilitates team meetings, as well as, encourages parent participation in these meetings that address the childs needs. * Manages and maintains caseload as outlined by the DOH-EIS and keeps participant records in accordance with Federal, State, and ESH requirements. * Attends, facilitates, and participates in staff and team meetings. * Provides services within natural environments (e.g. home, day care and community). * Attends, facilitates, and participates in transition meetings. Other Responsibilities: * Attend, participate and collaborate in organization as well as department meetings, training, volunteer (and practicum) programs and events * Assists with other department duties as outline by manager * Assists in maintaining a clean, hygienic, safe and childproof environment. Reports any safety concerns to the Program Manager. Equipment Used: * Program materials and template documents to carry out assessments or sessions * Computer software, Microsoft 365 applications and 10 Key Calculator * Multi-line telephone with intercom system, company issued company equipment (work cellphone, computer, Ipad) * General office equipment QUALIFICATIONS Minimum Education, Experience & Training Equivalent to: * Bachelors degree in a social service or education field (e.g. psychology, sociology, early childhood) or related field from an accredited college or university program. Masters degree (M.A.) in Social Work, Social Service, Education or equivalent is highly preferred. * Current State of Hawaii license in Social Work or commitment to obtain licensure within one year is highly preferred * One (1) year minimum experience in professional social work is highly preferred. * Experience working with children with special needs in the field of early intervention is highly preferred. Knowledge, Skills & Abilities: * Significant knowledge with developmental disabilities, familiarity with ADA and Individuals with Disabilities Educational Act (IDEA) guidelines, and inclusion. * The ability, understanding and compatibility to interact with families, professionals, and paraprofessionals within a Primary Service Provider model of service delivery, using an approach that involves giving and receiving peer coaching. * Understanding of and respect for children with special needs and their families. Ability to be attentive to input from families regarding their childs preferences by utilizing a strength-based approach with culturally competent practices. * Positive public relations attitude and approach when interacting with those we serve and the general public. * Organizational ability to work independently and take initiative to complete assignments. * Clear and concise verbal and written communication skills and the ability to communicate effectively with people at all levels. * Operating knowledge of a computer, applications and software, standard office equipment and Microsoft (Outlook, Word and Excel). Must be able to use an iPhone or work cell phone if applicable. Mental Requirements: * Ability to work calmly with behavioral and health related incidents. * Alertness to respond to audio and visual cues from participants and their families, other staff members and the environment. * Ability to handle multiple tasks, adhere to timelines, and process urgent projects while maintaining an attention to detail. * Ability to respond appropriately and immediately to emergencies as defined by ESH Emergency Procedure Guide/Safety Manual. * Logic and reasoning for forecasting future needs, goals, and events. Physical Requirements: * Frequent bending, reaching, squatting, kneeling, twisting in order to observe, assess, and interact with clients. * Ability to sit in office chair and typing for periods of up to one hour without a break. * Ability to sit on the floor in families homes to administer evaluations that can last up to 90 minutes. * Occasional lifting, carrying, and loading/unloading toys and materials up to and including 25 lbs. to 50 lbs. used in home visits. * Visual and auditory ability to work with clients, staff and others in the workplace continuously. * Frequent speaking and listening (50%) to clients, staff, and other professionals in meetings and on the phone. * Frequent driving (to and from office and client homes). * Frequent exposure to disability disorders in clients; occasional emergency situation; occasional exposure to trauma; constant client contact and decision making; constant concentration required when working with clients. * Frequent work inside client homes; occasional work in outdoor settings. CONDITIONS OF EMPLOYMENT * Ability to successfully clear and maintain background check clearances through ESH's partnering vendors: * FBI Criminal reports * Department of Justice, Hawaii Criminal Justice, Office of Inspector General, National Sex Offender reports in accordance with Federal, State, and Local laws * Drug Test * Reference * Education * Valid State of Hawaii, Association certification * Ability to travel to multiple work sites and client homes: * Valid drivers license * Current auto insurance for vehicle used as means of reliable transportation * Must clear annual TB testing based on company policies * Valid First Aid and CPR certificate or card Easterseals Hawaii is an equal opportunity employer
