Primary Purpose: The Social Service Coordinator will complete the
duties of Social Services Department. Ensure that each resident
receives psychosocial support and person-centered care. The Social
Service Coordinator will focus on Mood, Behavior, Discharge
Planning, Psychoactive Medication Use, Abuse, Resident Rights,
etc.
Essential Duties/Functions:
- Review and explain advanced directives and resident's rights
and responsibilities to the resident and/or responsible party,
during the admission process or as requested
- Participates in the development of Care Plans for residents
related to Social Services
- Assist in the placement of residents through resident
assessment and coordinated discharge planning due to changes in
level of care, or in cases of lateral transfer
- Determine the type of assistance needed by the resident to
solve problems and assists the residents and family members in
doing so
- Assist in completing necessary forms and making arrangements
for assistance whenever indicated and involves the family or
responsible party whenever desirable
- Provide information to resident and his/her family in regards
to Medicare, Medicaid, and other welfare programs available under
Social Services, as requested
- Coordinate Notice of Medicare non coverage with IDT
- Complete the assigned sections on the MDS (Minimum Data Set)
within deadlines
- Ensure residents have all required documents on file
- Participates in the development of Policies and Procedures
relating to Social Services
- Maintain up-to-date documentation of all contacts made on
behalf of residents
- Receive grievances and recommendations from residents,
residents' families, and staff pertaining to policies and services
of the facility's staff and follow-up with appropriate
parties
- Investigate alleged abuse and document and report up the chain
of command
- Complete and fax discharge/transfer notice to SLTCO (State Long
Term Care Ombudsman) by due dates
- Follow-up with resident/family/POA with bed holds as described
in our Bed Hold Policy
- Comply with documentation requirements in accordance with
company policies and procedures and rules and regulations
Qualification Requirements:
- At least one (1) year of related experience
- Must have Master degree in Social Work or Licensed Clinical
Social Worker and must be in good standing
- Must have current CPR and First Aid Certification
- Must be able to assess and evaluate care needs of
residents
- Must be able to manage difficult situations with residents,
family members and/or POA
- Must be able to collaborate with members of different
departments
- Must be able to communicate verbally and in writing with
others
- Must be able to use technology and/or office equipment to
complete work assignments
- Must be thorough in documenting information and follow-ups with
work assignments
- Must be organized and able to complete assigned duties within
strict timelines
